<p>According to Abraham Lincoln, &#8220;I am a firm believer in the people. If given the truth, they can be depended upon to meet any national crisis. The great point is to bring them the real facts.&#8221;</p>
<p>America, you, the people, need to decide.</p>
<p>Health care has two primary elements: products and services. Products are easier to analyze than services, so let&#8217;s focus there.</p>
<p>Health care products include items like crutches, wheelchairs, hearing aids, eye glasses, artificial hips, splints, medications, lab tests, etc.</p>
<p>We all understand that manufacturers need to make a profit. If it costs $5 in raw materials to manufacture a jacket, we don&#8217;t argue when they sell it wholesale for $10. We understand they have to cover their development costs, their labor, their rent. Nor do we argue when retailers mark the price up to $20 &#8211; they, too, need to cover their overhead &#8211; and hope to make a profit as well. The mark-up also allows retailers to discount their price if desired, and is a cushion against inventory losses, shoplifting, etc.</p>
<p>But how is it that a drug that costs $4 this year may have been $100 last year? Why do hearing aids cost $3000 when a home computer is $300? How can over-the-counter reading glasses cost $2 and prescription glasses $200? What does it cost to manufacture an artificial hip?</p>
<p>A curious thing happened today. I received a notice from one of the two local hospital systems regarding updated lab test pricing. Of course, I expected the prices to be higher. Lo and behold, the prices had been cut &#8211; chopped, in fact &#8211; some by as much as 70%! The reason? They desired to be more competitive in the local market.</p>
<p>Now that&#8217;s one example of the free market at work. If certain medical items were deregulated &#8211; hearing aids, for example &#8211; watch how fast the price would drop. The situation is complex and could not be solved overnight, but with a little ingenuity, it could be solved!</p>
<p>There are too many questions to be addressed in a brief discussion, but let&#8217;s address just one: hearing aids. Perhaps there was a time when the industry required FDA regulation, but I would argue, that day is past. In this era of cell phones and iPods, the free market could manufacture inexpensive hearing aids with various settings, using online hearing evaluations. In fact, why not have Verizon get in the business? With the computer-literate baby-boomer generation reaching Medicare age, in a free market the problem of $3000 hearing aids would take care of itself.</p>

			<content:encoded><![CDATA[<p>The 76 million baby boomers retiring in the next 18 years will place a burden on the already weak Medicare system. I know I have been retired for 16 years and will be eligible for Medicare in 4 months.</p>
<p>I should be happy, I have had catastrophic ($10,000 deductible) health insurance for the last 15 years. I am /was looking forward to going to the doctor when ill&#8230; instead of taking aspirin and letting time do the healing. Medicare was looking good to me.</p>
<p>My current family doctor recently gave me the happy news he is dropping Medicare due to the reduced payments to doctors&#8230;wonderful! Think the 500 billion &#8220;health care reform&#8221; is taking out of Medicare will help matters? I do not think so.</p>
<p>The sad facts are that Medicare will be running out of money in 2019. It currently makes up 15% of the federal budget. There are currently 46 million Americans covered under Medicare. In twenty years time there will be 77 million on Medicare&#8230;a two-thirds increase into an already sick system.</p>
<p>Think the politicians will fix this? Medicare and Social Security are the third-rail of American politics&#8230;everyone realizes they must be fixed but no one wants to commit seppuku to do so personally.</p>
<p>Baby boomers should be aware of this situation. For those nearing retirement they should check with their current family doctor to see if they take Medicare and will they take Medicare in the future.</p>

			<content:encoded><![CDATA[<p>President Obama recently signed into law a new healthcare reform bill which will make expand health insurance coverage for 32 million who do not currently have insurance coverage. This law will be phased into effect over the next decade but it offers a lot to help family caregivers both in the short term and the long term.</p>
<p>The benefits of the law fall into 2 categories &#8211; provisions which help the care recipient through public programs and those that help the caregiver through expanded health insurance coverage and better consumer protections. The biggest advantage to care recipients comes through expanded Medicare prescription drug coverage and additional health insurance options for pre-Medicare retirees.</p>
<p>Family caregivers will see their biggest benefits come from the fact that they will be able to get health insurance whether or not they are employed. The new insurance coverage options will be a big benefit to caregivers because many remain tied to jobs in order to receive health insurance. By providing new coverage options and subsidies to help pay for them, family caregivers now have additional options on whether or not they want to continue working or care for their loved one full time. Below we walk through the key provisions which are likely to impact family caregivers.</p>
<p>Goes Into Effect Over the Next Year</p>
<p>The expansion of coverage provided by the new healthcare reform law will not be available until 2014 but there are many portions of the law which will go into effect during 2010 and 2011.</p>
<p>Improvements to Medicare Prescription Drug Coverage. The bill provides a $250 &#8220;bonus&#8221; to all people in a Medicare Prescription drug program in 2010 that hit the &#8220;doughnut hole&#8221; which currently begins at $2,830. It also begins closing the Medicare Prescription Drug &#8220;doughnut hole&#8221; in 2011 and would completely eliminate it by 2020.</p>
<p>Helping Sandwich Generation Caregivers. The bill helps sandwich generation caregivers, those caring for sick parents or relatives and their own children, by:</p>
<p>* Eliminating pre-existing conditions for children thereby ensuring sick children can get insurance coverage.<br />
* Permitting children up to age 26 to stay on their parents insurance policies regardless of whether or not they are full-time students. Most insurers currently cover dependents not in school only up to age 19 and those in school up to age 23.<br />
* Prohibiting lifetime benefits caps on health insurance.</p>
<p>Temporary Early Retiree Program. The law requires the creation of a temporary voluntary reinsurance program for employers to help cover early retirees within 90 days of enactment. This should help pre-Medicare retirees find affordable coverage. The program ends in 2014 when the expanded coverage provisions come online.</p>
<p>Medicare Advantage Changes. The law changes the way that Medicare Advantage plans are paid by bringing payments closer into line with the costs of the regular Medicare program. These payment changes may impact the benefits provided under these plans so keep review your plans coverage carefully at renewal time.</p>
<p>2011 and Beyond</p>
<p>The bulk of the new benefits and subsidies for health insurance will begin in 2014. The lead time is necessary to accumulate funds to pay for the cost of the new coverage.</p>
<p>Expanded Coverage Through Health Exchanges. These state-based marketplaces start operation in 2014 and are based on the Massachusetts Health Connector system, in which individuals and small businesses are able to purchase health insurance. The goal is to provide individuals and small businesses with transparency in benefits and pricing so they can compare health insurance and purchase the coverage that best fits their needs.</p>
<p>Individual Mandate. Almost all Americans will be required to get insurance coverage or face fines-a system similar to the one that&#8217;s already in place in Massachusetts. The fines start small at $95 in 2014, but rise rapidly to $695 in 2016. Low-income individuals are exempted from the mandate but most of them will be eligible for coverage through Medicaid or other subsidies.</p>
<p>Health Insurance Subsidies. In order to help families afford the mandatory insurance premiums, starting in 2014 subsidies will be available on a sliding scale, up to a level of $88,000 per year for a family of four. The amount of the subsidies will also be on a sliding scale, meaning that people with lower incomes will receive larger benefits.</p>
<p>Long Term Care. The bill includes the CLASS Act, which provides a public, voluntary long term care program that working people can purchase. The program would cover home care, respite care, home modifications, transportation, and assistive technologies.</p>
<p>What&#8217;s Next</p>
<p>While health insurance reform has passed and will be signed by the President within days, the Senate still needs to act on a bill to fix some of the problems in the bill enacted into law. That&#8217;s expected to take place this week.</p>
<p>It is unlikely that work on health reform is done. The lesson of other social insurance programs is that they always remain works-in-progress. Social Security and Medicare have been modified and expanded a great deal since they were first enacted. You can expect similar developments to occur with healthcare reform as it moves forward.</p>

			<content:encoded><![CDATA[<p>One of the most common phone calls I receive in the office is when someone&#8217;s mother or father is admitted to the hospital. In this time of crisis, answers are not easy to come by.</p>
<p>How does their health insurance work? What does Medicare pay for? Once the parent is discharged, what happens, where do they go, how is it paid for, what are our options? What do we do if mom or dad is going to have to go to a nursing home? How do we pay for it?</p>
<p>This confusion is expected as the senior health care system can be a very confusing and overwhelming process. The first thing to do is to understand the basis for today&#8217;s system.</p>
<p>In 1983, Congress created the Prospective Payment System. This is important because when a person 65 or older is admitted to a hospital, he is assigned only one of 473 Diagnostic Related Groups (DRG&#8217;s). This is important because Medicare compensates the hospital a flat dollar amount for the DRG assigned to the patient.</p>
<p>Let me give you an example. Say that my father is admitted to the hospital with lung problems and the DRG is four days. If my father is discharged in three days, then the hospital makes one day of profit. If my father is discharged in five days then the hospital loses money and cannot bill the patient for the one extra day.</p>
<p>Back in the good old days, I remember when my grandfather was in the hospital and the nurse asked him if he felt well enough to go home because if he didn&#8217;t, he could stay a few extra days until he felt better.</p>
<p>Today, it is all about the money. Once a patient is no longer getting better or worse, in other words, is deemed to be &#8220;stable&#8221;, then the patient is discharged either to home or a Medicare certified nursing home or rehab facility.</p>
<p>In order for Medicare to pay for rehab care the patient must have been in the hospital for three consecutive days (72 hours). Then, no later than thirty days after discharge from the hospital, be admitted to a Medicare certified nursing facility.</p>
<p>If these criteria are met, then for 2010, day&#8217;s one through twenty in the rehab facility are paid for 100% by Medicare. For days twenty one through one hundred, your co pay is for this year is $137.00 per day.</p>
<p>From day 101 and beyond, regardless of your condition, you are responsible for all of the facility costs.</p>
<p>Keep in mind, that in order for this reimbursement schedule to happen, you must either be getting better or getting worse. Like the hospital, once you are deemed to be stable, you come off the Medicare reimbursement schedule and must pay for all costs.</p>
<p>In California, most patients will come off of Medicare reimbursement around week three and must begin private paying from this point forward. The business office will advise you when this is expected to take place.</p>
<p>If the facility has long-term care beds, then the patient may be able to stay in the same facility. But if the facility is strictly short-term care or rehab, then the patient must find another facility or go home.</p>
<p>How does the patient&#8217;s health insurance fit into this? It all depends on what type of plan that the senior patient is on. Is it a Medicare supplement plan or PPO, or is it a Medicare Advantage plan like an HMO?</p>
<p>Medicare supplement insurance, also called Medigap, is private health insurance designed to supplement Medicare. A premium is paid for this coverage which is age rated.</p>
<p>There are twelve standardized Medigap plans, A through L. In most states, you can go to any doctor or hospital that accepts Medicare without pre-authorization. Under plans C through J, days one through twenty are completely paid for by Medicare. For days twenty one through one hundred, the Medicare co-pay for 2010 is $137.00 which is covered by the Medigap policy. From day one hundred one and beyond, the patient is responsible for the full cost.</p>
<p>For Medicare Advantage plans such as an HMO like Secure Horizons, SCAN and Kaiser, the patients may have a co-pay from day eleven of $100. It is best check the benefits booklet or call the customer service department.</p>
<p>If someone goes to a facility without going to the hospital first, then you must private pay from day one.</p>
<p>Once the patient comes off Medicare reimbursement, if qualified, Medi-Cal will help to pay for the nursing home costs. If going to the facility directly from home, then, if qualified, Medi-Cal may help to pay for the nursing home costs from day one.</p>

<p>As majority of people are working so it becomes impossible for them to stay at home and help their parents. So it is better to look for a home care company. So they can stay at their home and can get help without being shifted to hospital or some other place. Living at home helps to stay healthier, as they would not be subjected to all germs. They can have visitors whenever they want and without any restrictions of visiting hours. This keeps them attached to their family and loved ones. You are the advocate for your loved one and you need to be sure that all of their medical needs as well as personal needs are met. It is important to learn about the resources that you have available to you right in your community. You do not have to do all of this alone as there are resources and support groups to help you when it comes to elder care.</p>
<p>There can be many reasons that your loved one is in need of senior health care such as: they suffered a broken hip where rehabilitation is a necessity, they are suffering with dementia or Alzheimer&#8217;s and are in need of constant care or maybe the effects of aging is limiting their ability to do things on their own. No matter what the cause is for the need of elder care, the fact is that you need to be cautious when it comes to senior health care. You really need to be aware of how they are caring for the person you are responsible for. If you put yourself in the senior&#8217;s shoes, it is easy to know what to look for. Are they feeding him or her right? Are they spending time with your loved one? Is your loved one&#8217;s needs being met?</p>

There is a great deal to remember when you are looking after an elderly loved one and you do not want to miss anything that is necessary for their well being. Most likely you are working with a limited budget so finding quality senior health care can be difficult, you have to make the most out of what you got.</p>
<p>It often becomes necessary to lean on other resources to help you in this matter. There are plenty of free resources as well as ones that are covered on various types of medical insurance including Medicare that can help keep you focused on what needs to be done. It can be very helpful for you to make a checklist of all things that need to be managed where senior health care is concerned this way nothing will be missed.</p>

If your loved one is staying at home, it is very crucial to be on top of their medications so that they do not miss a dose of a crucial medication or so that they do not over medicate themselves.</p>
<p>There is a great deal to remember when keeping up with senior health care but once you get all of the necessary information you need, have a check list of what your responsibilities are and a resource list of who you can turn to for help, you will find that everything will fall into place. You will have a better understanding of what you need to do and that will make staying on top of all of much easier. Remember that you are the advocate for your loved one and in some cases you will be the one taking care of all of their needs and all that goes with senior health care. Remember that you have a resource list in place to help you with this so you will not get overwhelmed or run down when you use the resources as needed.</p>
